+P ammunition (hot-loaded, overpressure)
+P ammunition is ammunition that has been charged with more powder than the standard specifications for that ammunition type. This creates higher bullet velocities, resulting in flatter trajectories and increased terminal performance (lethality). +P ammunition usually has more recoil, due to the higher muzzle velocity.
+P is an official technical designation for overpressure rounds issued by the Sporting Arms and Ammunition Manufacturers' Institute (SAAMI), so a '+P' label on a box of ammunition indicates that that ammunition has been over-loaded according to approved SAAMI overpressure specifications. By contrast, "hot-loading" usually refers to unregulated production of overpressure ammunition by individuals who hand-load their own cartridges. Hot-loading will usually void a firearms warranty, as will using +P ammunition in a firearm that is not designed to withstand +P ammunition.

+P+, ++P ammunition
Some manufacturers have created "+P+" or "++P" loads, which are loaded beyond the SAAMI overpressure specifications. However, +P+ is not an official or regulated designation, and is not recognized by SAAMI or SAAMI's European counterpart, the CIP.
